Payment Policies PAYMENT IS REQUIRED THE DAY SERVICES ARE RENDERED: With or without insurance, payments are expected at time of service. We gladly accept cash, check, American Express, Discover, Mastercard, and Visa. Our practice offers a flexible payment program called CareCredit.
FINANCING: CareCredit is a revolving charge account that is easy to use. It takes just minutes to apply, and is ideal for all treatment costs not covered by insurance. Whether you have insurance or not, CareCredit can help you afford the treatment you need. You can go online and apply at www.carecredit.com at any time. Ask our receptionist or office manager for an application or help with applying for CareCredit.
INSURANCE: We are happy to submit dental claims for you. Even with insurance, you are still responsible for any account balance and/or estimated co-payments. Please advise our receptionist or office manager of any changes in your dental insurance so claims can be submitted accurately.
WE ARE PARTICIPATING PROVIDERS FOR: Anthem Federal, Careington, Delta Premier, Dental Care Plus, Denta Select, and Superior. If you do not see your insurance above, don't worry. We may not be on your plan, but we can still accept your insurance. Just because we are not contracted, does not mean we can not see you as our patient. We will accept and bill all dental insurances. Feel free to contact your insurance company to find out your coverage for our office. If you still have questions about your dental coverage please ask our office manager.
OVERDUE AND UNCOLLECTED ACOUNTS: Accounts that carry a balance past 30 days will incur an interest fee of 2%. Statements that have been ignored or accounts 90+ days past due will result in collection action. We will turn over any uncollected account to Doctors Credit Service, Inc.
NOTICE: We do charge a missed appointment fee of $40.00 if you fail to give 24 hour notice.
